PHP Case Manager

POSITION SUMMARY:
Under the direct supervision of the Program Director, this position provides appropriate activities for intakes, intervention, support, referral to treatment and monitoring of physicians, physician assistants, medical students (program participants), chiropractors and any other allied group participants with health related issues such as substance use disorders, mental and emotional concerns, physical illness, behavioral health issues and other issues resulting in PHP involvement. Functions as PHP’s liaison with designated hospitals and clinics in matters concerning the monitoring of program participants. Meets regularly with program participants to ensure that participants adhere to Monitoring Agreements, Professional Development Plans and Consent Orders when applicable.

DUTIES AND RESPONSIBILITIES:
  • Meet with assigned referrals, make initial assessment, and refer to appropriate staff and/or facilities for evaluations as needed. Participate in Feedback and Recommendations for Treatment and Aftercare Plans.
  • Monitor contracting physicians, physician assistants, medical students, chiropractors and other allied groups in caseload, conducting telephone check-ins and in-person follow-ups on a regular scheduled basis and ensures compliance with PHP monitoring requirements as defined in PHP monitoring agreements/ Consent Orders/ Professional Development Plans and Informal Monitoring.
  • Perform consultation liaison activities as needed with hospital staffs to familiarize them with the work of PHP and to provide support around specific interventions with participants or potential participants.
  • Work with the Kansas State Board of Healing Arts Staff and other Medical Boards as necessary to provide required monthly or quarterly reports as required by Consent Orders and Professional Development Plans.
  • Monthly and Quarterly reports to PHP’s, hospitals, clinics and credentialing groups.
  • Document, in a timely fashion, all participant discussions and activities through progress/case notes using PHP Data Base.
  • Work closely with PHP Testing and Monitoring Coordinator to keep information and testing updated and accurate at all times.
  • Assist in establishing and maintaining support groups for participants with behavioral issues.
  • Remain available on a PRN (alternating on-call time with the Program Director) basis to coordinate the confrontation/intervention of any potential participant or contracting physician who is experiencing a relapse or clinical need. This may involve contacting other staff members and/or working with the PHP Program Director to determine an appropriate course of action.
  • Communicate participant’s progress during weekly meeting with Program Director, as applicable. Needed information will be shared in monthly meetings with the team.
  • Assist with research projects and developing educational materials.
  • Work in conjunction with the KMS-PHP team on short and long-term projects as directed.
  • Handle confidential and non-routine information discretely.
  • Other duties as assigned.
QUALIFICATIONS:
  • Bachelor’s Degree preferred or five years’ experience with a background in psychology, social work or other behavioral science, PHP or toxicology.
  • Familiarity and experience in handling substance use disorders, behavioral, emotional, mental health issues.
  • Must have high level of interpersonal skills to handle sensitive and confidential situations. Position continually requires demonstrated poise, tact and diplomacy.
  • Must have ability to work well in emergency situation and provide immediate assistance in crisis situations.
  • Must have ability to assess and prioritizes the needs of the participant/potential participant and keep accurate records of case notes.
  • Work requires strong communication skills both written and verbal.
  • Work requires ability to handle working with other agencies and professionals to ensure that participants are compliant with monitoring.
  • Ability to advocate on behalf of participants who are in compliance. This may entail testifying at the Kansas State Board of Healing Arts and Administrative hearings, meeting with hospital/clinic professionals.
  • Capability of managing confrontational professionals who are impaired, not compliant or oppositional.
  • Ability to maintain confidentiality.
  • Ability to work with a diverse group of different professionals (i.e. attorneys, administrators, risk managers, psychiatrists, therapists, cultural diversity, etc.).
  • Ability to work independently yet be an integral part of the PHP staff as a team player.


WORKING CONDITIONS:
Professional office environment. Contact with medical office staff, physicians, attorneys, and general public. May deal with angry or upset people and individuals with emotional or mental health issues. Multi-task position requiring strong attention to detail.
