The Missouri Primary Care Association (MPCA) is a non-profit organization, which explores and implements activities aimed at providing and promoting high quality, accessible health care services to underserved urban and rural populations in Missouri.
Primary Accountability:
The Quality and Performance Improvement Coordinator is an individual who works with outpatient primary care practices on patient-centered quality of care, practice transformation, team-based care, chronic/preventative disease management, patient safety and risk management. This position works in collaboration with internal staff members and/or departments as well as external partners in the development, implementation, and evaluation of projects and duties.
Major Duties:
  • Provide customized facilitation/coaching to organizations participating in the Primary Care Health Home Initiative by working with the sites to identify site‐specific needs and opportunities, prioritizing areas of focus, and identifying available and needed resources.  
  • Assist member health centers and partners with quality improvement, including the development of quality improvement plans, performance improvement, practice and workflow redesign, troubleshooting, and strategic planning regarding data quality/integrity to optimize the collection and use of data.
  • Provide training and technical assistance to member health centers regarding risk management, patient safety, and peer review best practices, monitoring, and reporting
  • Provide training and technical assistance to member health centers, outpatient primary care, and partners/stakeholders regarding patient-centered care using team-based approaches and strategies
  • Actively participate in the Population Health Management activities of the Association
  • Leverage data system functionality to advance quality improvement, care that is patient-centered and team-based, patient safety to enhance, improve, and monitor clinical, financial, and operational outcomes.
  • Manage assigned grant program activities and objectives including reporting to funding agencies and creating ongoing opportunities for health center staff to connect, creating technical assistance and training opportunities.
  • Work collaboratively with other staff for reporting, training, and program coordination.
  • Coordinate specific projects among member and partner organizations, including State agencies as assigned.
  • Perform additional duties as assigned
  • Bachelor’s degree in nursing, public health, health care administration, or other related health care field
  • Registered Nurse preferred, Licensed Practical Nurse with 10+ years experience considered
  • 5+ years experience working in a Federally Qualified Health Center or outpatient primary care practice highly preferred
  • Training and/or certification in quality improvement, risk management, patient safety, patient‐centered medical home is highly preferred.
  • Must have a valid Missouri Drive License, be able to use personal automobile and maintain all liability and other automobile insurance as required by law.
  • Strong knowledge of and the ability to effectively utilize all Microsoft Office Applications.
  • Ability to learn additional software applications.
  • Ability to work in a team environment and show flexibility and versatility.
  • Strong interpersonal communication skills and the ability to establish and maintain effective working relationships with internal and external customers
  • Excellent communication skills including oral, written, and public presentation.
  • Ability to effectively present information in various formats such as one‐on‐one, small group, or large group settings.
  • Strong organizational skills and the ability to prioritize, coordinate and manage multiple projects and workload responsibilities with minimal supervision.
  • Ability to work with strict confidentiality requirements and utilize good judgment in the preparation and distribution of confidential information.
Physical Demands:
  • Ability to sit or stand for extended periods of time.
  • Ability to read computer screens, mail, e-mail and talk on the phone.
  • Ability to lift up to twenty-five (25) pounds.
  • Ability to drive a vehicle.
  • Some overnight travel may be required.
 Work Environment:
  • Professional, team, and deadline-oriented environment in an office setting.
  • Interaction with internal and external customers.
  • Main office located in Jefferson City, MO (remote work locations will be considered, but will need to travel to Jefferson City frequently)
